If you've had unexplained dizziness or vertigo alongside jaw symptoms, and your doctor has ruled out inner ear infections, BPPV, or other vestibular causes, TMD is worth putting on the list of possibilities — though it's one of the less understood connections in this space.
The proposed mechanism
Two small muscles — the tensor veli palatini and tensor tympani — help regulate pressure and tension around the middle ear, and both sit close to structures involved in TMJ function. When TMD causes chronic muscle tension or joint inflammation nearby, it may affect these muscles' normal function, altering middle-ear pressure and fluid dynamics in a way that sends confusing signals to the brain's balance system.
There's also a proposed mechanical pathway: the temporal bone, which houses part of the inner ear's vestibular system, sits directly adjacent to the jaw joint. Chronic joint misalignment or muscle imbalance in this area could theoretically place subtle mechanical stress nearby, though this is a less-studied hypothesis than the muscular one.
How to tell if TMJ might be a factor
- Standard vestibular and neurological workups have come back normal
- Dizziness correlates with jaw symptoms — worse during flares, better when the jaw is calm
- You also have other TMD signs: clicking, soreness, limited opening, or a history of clenching
- The dizziness is more of a vague unsteadiness than the room-spinning sensation typical of inner-ear vertigo (though some people do report true vertigo)
